WORK CONTRACT

PURPOSE:

The purpose of this work contract is to identify the Obligation of CCTV Contractor __________________________ and the client_______________________ with regards to Purchase, Installation, and Warranty.

REFERS TO QUOTATION/PROPOSAL OR P.O.

SCOPE OF WORK DURING INSTALLATION:

A. The CONTRACTOR (CCTV Provider) ______________________ will provide

-          To provide CCTV Equipment and solutions as quoted
-          Camera cabling and power wiring
-          Termination of head end
-          Product Demo, Testing & Configuration

B. CLIENT TO PROVIDE:

-          Assistance with the help of in-house technician OR maintenance (if available) for electrical tapping and for other safety purposes concern.
-          To provide Router and internet connection or application of Internet provider for use of Network Access feature.
-          To provide space or space allotment for DVR safekeeping.
-          To provide any necessary work permit if any is required in their subdivision, home or office.
-          To provide for any materials required/requested that is not in the quotation or bill of materials.

TERMS OF PAYMENT

A. 50% Downpayment upon signing of quotation / P.O.
B. 50% Upon completion of installation and training.

CANCELLATION

50 will be forfeited in the benefit of the supplier for import charges survey, demo, lay-out & representation expense.


WARRANTY & SERVICE

·         Mechanical and electrical failures that occur during normal use of your item. Your item is covered for up to the full purchase price of your item.
·         Mechanical Breakdown – inability of a properly maintained part to perform the function for which it was designed, due to defects of workmanship (Mechanical Breakdown DOES NOT MEAN the gradual reduction in operating performance due to wear and tear).
·         REPAIR COST – in case of repair, the cost is determined by the dealer’s regular prices up  to the Manufacturer’s Suggested Retail Price.

Warranty Period Begins on the Completion date of Installation up to the 12th month period: 1 year on Hard Drive and not including Mouse / Remote control.

Tampering on the items/units VOIDS this Warranty.

Warranty does not extend to cover any damages or malfunction resulting from environment disaster, accidents or mishaps abnormal humidity / temperature, improper voltage, misuse, negligence, ignorance, molds or modifications made by any person (s) other than supplier’s authorized personnel or representative.

Client shall be responsible for backing up the data contained in hard disk drive.

Network is NOT PART of our service. Any internet related or connection please check with your internet provider. I.T. Assistance can be provided with minimum charge of Php500 per service.

Having a Static IP for network access of DVR is an advantage for client.

B. SERVICING REQUEST
Service schedule is within 24-48 hours after receiving Job order number, to be provided when you call or email for a schedule. Client may issue a service request via, fax, telephone or email and must receive a confirmation / job order number for the service requested.

Only minimum gasoline P350 is charge for every service request on the first year to be paid directly to the technician who will service you.

C. REPAIR

1.    Minor Repair: A Minor repair includes Checking, cleaning and adjusting on reported defects. There may be additional charges for replacement parts.
2.    Major Repair: A Major Repair includes Checking, Cleaning, adjusting and testing of all minor and major assemblies. Major repair may vary depending on the difficulty of repair and replacement parts needed.
3.    Transfer of camera to other location after the installation will be charge to the client accordingly.
4.    Beyond Economical Repair: Unit will be returned un-repaired due to the repair price exceeding the price of the item.

FINAL ACCEPTANCE

Upon signing below, Client signifies Final acceptance of Delivery, Installation and that the following training and demo was provided and explained to them in the manner that they understand.
